چکیده

The year 1882 was an annus mirabilis in the history of pulmonary tuberculosis, for it saw not only the discovery of the tubercle bacillus by Robert Koch but also the introduction of artificial pneumothorax treatment by Carlo Forlanini. At that time Koch's spectacular achievements and the exaggerated expectations raised after the introduction of tuberculin tended to overshadow the significance of Forlanini's contribution to the management of pulmonary tuberculosis. In modem times drug treatment has eclipsed the former methods of collapse therapy, but for some 40 years before the introduction of antituberculous drugs artificial pneumothorax offered hope and in many cases cure to tuberculous patients around the world. The centenary of the start of Forlaninis pioneer work on collapse therapy in pulmonary tuberculosis provides a suitable occasion to pay tribute to this great Italian phthisiologist (fig 1).

Bolshevik Disease and Stalinist Terror: On the Historical Casuistry of Artificial Pneumothorax

From its initial development by Carlo Forlanini at the end of the nineteenth century until the advent of antibiotics in the 1940s, artificial pneumothorax was one of the most widely used treatments for pulmonary tuberculosis. However, there were strongly held reservations about this therapy because of its risks and side effects. In the Soviet Union under Stalin, such uncertainties became instru...

Revisiting an old therapy for tuberculosis.

Artificial, therapeutic pneumothorax was commonly used to treat tuberculosis (TB) prior to the development of antimycobacterial agents. Recent reports1-2 suggested that therapeutic pneumothorax is efficacious. In 1820, James Carson suggested therapeutic pneumothorax for TB,3 and Carlo Forlanini began to apply it in clinical practice in 1888.4 Therapeutic pneumothorax evolved from the idea that ...

Improving tuberculosis surveillance in Europe is key to controlling the disease.

It was already well known since the sanatoria period (Germany, 1857) when treatment against TB consisted of good food, rest, sun, and fresh air that about half of TB cases recovered almost spontaneously. Robert Koch’s discovery of Mycobacterium tuberculosis in 1882, Carlo Forlanini introduced the artificial pneumothorax in 1907 [1] and streptomycin was introduced at the end of the Second World ...

Closed intrapleural pneumonolysis.

In 1882, Forlanini12 of Milan proposed the induction of artificial pneumothorax in patients suffering from pulmonary tuberculosis, and in 1894’s he published his experience with the first patients to be submitted to this form of therapy. In 1885 Cayley had reported the case of a tuberculous patient in whom he had induced pneuniothorax by open incision of the chest wall to control hemoptysis, an...

A plea for increased caution in the use of surgical collapse therapy for pulmonary tuberculosis.

Not many years ago-within the lifetime of some of us-pulmonary tuberculosis was considered an almost hopeless disease. Patients were advised to “go out West and rough it,” and those who did recover did it in spite of this advice rather than because of it. Examples of the fatalistic attitude prevalent in those days are too well known to all of you to merit discussion at this time. In 1877, what ...
